ADMINISTRATIVE ASSISTANT HISPANIC APOSTOLATE, YOUTH & YA MINISTRY, MIGRANT MINISTRY -  PASTORAL CENTER 

The Diocese of Stockton has a full-time, non-exempt administrative assistant position available.

This position provides bi-lingual administrative and clerical support for the offices of the Hispanic Apostolate, Hispanic Youth and Young Adult Ministry and Migrant Ministry.   Major responsibilities include responding to inquiries in a timely manner, prepares, edits, and proofreads various correspondence, assists with program events, including designing layouts for flyers, etc., assists as backup receptionist for Pastoral Center as needed.   Qualifications: high school degree or equivalent, 4-5 years of successful office experience, experience with Excel, Publisher and Microsoft Word, excellent verbal and written bi-lingual skills (English and Spanish). Program: Multipurpose Senior Services Program (MSSP)

Position: Supervising Care Manager (SCM)

Job Type: Full-Time 40 Hours per week

Salary: $32.00 -$34.25 /hr.

General Description:

Under the direction of the Site Director, the SCM is responsible for planning, organizing, developing and implementing the principles and techniques employed in a comprehensive care management model program for MSSP older adults.  The SCM position combines both administrative duties and clinical supervision responsibilities.  The SCM is responsible for providing clinical oversight of four Social Woker Care Mangers (SWCM) to ensure all care management activities are met for 200 participants monthly and ensures documentation meets CDA Standard Agreement Requirements and Waiver requirements. While both areas have to be balanced, it is important that the time required for direct clinical supervision of the SWCM’s not be eroded by administrative and managerial functions.

Master’s Degree required in socialology, phychology, gerontology, rehabilitation, counseling and two years experience working with older adults (60 years and Over).

Program: Catholic Charities’ Multipurpose Senior Services Program (MSSP)

Position: Nurse Care Manager (NCM)

Salary: 46.35/hr. to $47.28/hr. Part-Time

Seeking a Nurse Care Manager (NCM) to join our Multipurpose Senior Services Program (MSSP). This is a Part-time position (25 hours per week). The NCM is responsible for conducting in-depth assessments/reassessments of participant's medical, physical and health needs to ensure eligibility for Medi-Cal. The NCM works closely with Social Worker Care Managers (SWCM) and certifies the level of care of assigned participants. Job responsibilities include:  physical and cognitive assessments as necessary and interpreting clinical findings. Works closely with the Supervising Care Manager, Social Work Care Managers, Physicians, and other community healthcare professionals. Certifies level of care, together with SWCM’s develops individualized care plans, monitors participants needs and provisions in the care plan, updates participant case file on a timely bases. NCM must be willing to travel throughout the San Joaquin County to work with participants in the community. Desired qualifications: Must possess a current CA RN License and in good standing. Experienced working with seniors and case management. Must be able to communicate effectively both verbally and in writing using online database.

Position: Social Worker Care Manager (SWCM)

Catholic Charities Diocese of Stockton’s Multipurpose Senior Service Program (MSSP) has an opening for a Full-time Social Worker Care Manager (SWCM) position, 40 hour week. The MSSP SWCM provides a wide variety of care management services to a diversified participant population of older adults living in San Joaquin County utilizing a comprehensive psychosocial services model. MSSP provides in-home care management and coordination to participants who are 60 and older and certified eligible for a skilled nursing home. The position requires the SWCM to conduct assessment/reassessments, and case progress notes for 50+ cases each month. Interacts with physicians, medical specialists, vendors, etc.  Identifies and develops support systems for the participant.

Tribunal Director-Diocese of Stockton Pastoral Center
The Diocese of Stockton is seeking a full-time Director of the Tribunal. (35 hours per week) Primary responsibilities include managing the daily flow of work in the Tribunal (as stipulated in Dignitas Connubii, art. 61 §2), supervising and evaluating its staff, managing documentary and administrative processes, and handling requests for matrimonial dispensations and permissions.
Required Qualifications:

1. Fully initiated member of the Catholic Church.

2. Licentiate or doctorate in canon law.

3. Commitment to the ecclesial vision and mission of Vatican II.

4. Ability to maintain confidentiality.

5. Excellent written communication skills.

6. Valid California Driver's License.

7. Knowledge of Spanish is helpful.

ST. PATRICK ANGELS CAMP - OFFICE MANAGER

St. Patrick’s Church, Angels Camp has a part-time, non-exempt position available for an Office Manager.

ALL SAINTS/ST. JOSEPH PARISH - TWAIN HARTE

All Saints/St. Joseph’s parish in Twain Harte and Tuolumne is looking for an independent contractor who is a seasoned musician and cantor for their Sunday masses.  This individual should be familiar with liturgical settings and hymns, but also can be trained by the Music Director.
